Overview

This policy explains what information Waterview Dentistry collects through this website, how we use it, and your choices. It covers this website only. How we handle your protected health information as a dental provider is described in our Notice of Privacy Practices, included with our online new patient forms and available at our office.

Information you give us

When you use our contact form, we collect what you enter: usually your name and phone number, and optionally your email, reason for visit, preferred day and message. It is emailed to our front-office team so they can follow up, and it is used only to respond to you. We don't sell it or share it with unrelated third parties.

Please don't send detailed medical information or describe a dental emergency through the form. It isn't a secure channel for clinical information and isn't monitored around the clock. For anything urgent, call (972) 346-5133.

Sending the form doesn't confirm an appointment. Our team will contact you to arrange one.

Online appointment requests and patient forms

The "Request Appointment" buttons and our online new patient forms are run by our patient-communication provider, Yapi. Information you enter there is encrypted, handled under that provider's privacy and security practices, and sent to our office to arrange and prepare for your visit.

Information collected automatically

We use Google Analytics to count visits and which links people use, such as calls, directions, appointment requests and form submissions, so we can keep the site useful. It uses cookies and collects standard technical data such as your browser, approximate location and the pages you view, under Google's privacy policy. We never send what you type into our forms, or any health information, to analytics. We don't use advertising cookies. Pages that show a map embed Google Maps, which may set its own cookies under Google's privacy policy. Our hosting provider keeps limited technical logs, such as IP addresses, for security and reliability.

Retention

We keep form submissions and related correspondence only as long as reasonably needed to respond to you and keep accurate records, in line with applicable recordkeeping requirements.

Your choices

You can always contact us by phone instead of using the form. To ask us to delete information you've sent through this website, call or email us and we'll take care of it.
